Alexander Lee “Alex” Formica, 32, of Sawmill Road, Lake Katrine, NY died suddenly early Tuesday morning, May 19, 2020 having lost his long battle with addiction and the power it had over him. He was born in Kingston NY, on October 1, 1987, the son of Philip and Lynn (Schabot) Formica of Lake Katrine. Alex had been employed for the last three years as a salesman for Colonial Subaru and had worked in sales at other retail businesses in Kingston and in Albany. He was a 2005 graduate of Kingston High School and went on to graduate Ulster County Community College with a degree in Criminal Justice “with distinction”. He loved all sports and was very good on a jet-ski, skateboard or snowboard, in golf and in basketball. He enjoyed fishing in deep water, lakes and rivers. Alex loved running and ran in many Kingston Classics, Shamrock runs and around the neighborhood. He completed his first half-triathalon at Williams Lake in Rosendale. He was a life-long member of the YMCA of Kingston, and a long-time member of the Williams Lake Beach Club where he had many friends and good times. Alex had a big heart for his family and friends. He was a caring, generous and loving man. He always had a gift of touching people’s lives and didn’t have a bad word to say about anyone. He spent five years in the Hudson Valley Youth Volunteers and Ulster County Youth Council working programs to help the aged seniors of Ulster County and the Special Olympics in Saugerties. He was a member of the Reformed Church of the Comforter and especially enjoyed the Saturday night contemporary services. In addition to his parents, he is survived by his sister, Aris Henderson of Saugerties, and his brother Jamie (Kim) Ladd of Catskill. Also surviving are his paternal grandparents, Enrico and Claire Formica of Albany, NY, nephews Colton and Boden Ladd of Catskill and his niece, Adrianna Alexander of Saugerties, Aunts and Uncles, Mary and Charlie Redlich, Pete and Pam Formica, Margaret and Rob Muenkel, Ann and Tom Krause, Sarah Formica and Jason Varnell, Sue and Dan Nelson, Patty and Mike Sawick, Bill and Amy Schabot, and Bruce LaTorre, and cousins, Jessica (Shawn) Maggiore, Sara LaTorre, Eric and Jeff Nelson, Nick and Marissa Sawick, Jess, Bri and Dan Muenkel, Jeremy and Sam Redlich, and Gabby and Zach Formica. He is predeceased by his maternal grandparents, William and Mary Lou Schabot, and Aunts Leslie LaTorre and Kelly Schabot. Due to the current virus pandemic, funeral services will be held privately. Alex is entrusted to the care of the Gilpatric-VanVliet Funeral Home, 339 Broadway, Ulster Park in the Town of Esopus. A Celebration of Life for Alex will be shared at a later date. We hope that all his family and friends will come to share stories at that time. My Tribute Gift Foundation makes this online charitable giving portal web page MyTributeGift.org (the “Website”) available to you subject to your acceptance of and compliance with these terms of use (the “Terms of Use”). Your donation will be processed through My Tribute Gift Foundation for distribution to your selected charity. Your credit card transaction record will appear as My Tribute Gift Foundation. An official tax receipt for your donation will be issued by My Tribute Gift Foundation. Donations are sent out to charities at the end of each month for all donations received in that previous month. So a donation made on the first day of the month would be sent at the same time as one made on last day. Payments would be sent either by paper check or direct deposit. Please note that the there is 3% + 35¢/transaction fee for credit card processing, My Tribute Gift Foundation collects a 5% administrative fee. These fees help fund the cost of processing all donations, issuing tax receipts, and sending all appropriate information to the selected charity. These fees are assumed by your selected charity, but you will receive a receipt for the full amount of your donation. There are NO additional costs the charity has to endure to participate and accept donations through MyTributeGift.org. According to the American Institute for Philanthropy (charitywatch.org) a charitable organization is considered “Good” if they are spending less than 30% on administration and fundraising costs. My Tribute Gift Foundation helps reduce those administrative costs for charities and allows donors to contribute to lasting memorials for their loved ones.
When donating online, if the donation form has an option to add a tip, there is no convenience fee. The tip is optional and can be adjusted before you submit your donation. The amount of the tip will be displayed to you before you confirm the charge to your payment card. Tips are in addition to the donation amount, so they do not affect the amount that goes to the charitable organization. Under IRS and CRA rules, tips are not tax deductible to the donor.
You warrant and represent that all information and materials you provide to My Tribute Gift Foundation, through the Website or otherwise, are accurate and truthful and do not violate third party rights. Indemnity. YOU AGREE TO INDEMNIFY AND HOLD HARMLESS THE FOUNDATION PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, ACTIONS, SUITS AND PROCEEDINGS BROUGHT BY YOU OR ANY THIRD PARTY (COLLECTIVELY "CLAIMS") ARISING OUT OF YOUR USE OF THIS SITE IN BREACH OF THESE TERMS OF USE OR THE INFRINGEMENT BY YOU OF ANY INTELLECTUAL PROPERTY RIGHTS OF ANY PERSON OR ENTITY, INCLUDING ALL LOSSES, LIABILITIES, DAMAGES, COSTS AND EXPENSES (INCLUDING ALL LEGAL FEES) ARISING FROM SUCH CLAIMS.
If in any part of these Terms of Use is held, by a court of competent jurisdiction, to be invalid or unenforceable, such part of these Terms of Use shall be eliminated to the extent permitted in that jurisdiction, for purposes of that jurisdiction, and the remaining Terms of Use shall otherwise remain in full force and effect.
These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and all federal laws of Canada applicable therein. The courts of the Province of Ontario shall have exclusive jurisdiction over any dispute arising out of these Terms of Use or your use of the Website.
These Terms of Use constitute the entire agreement between you and My Tribute Gift Foundation with respect to the Website, except as we may explicitly agree otherwise in writing.. No delay or failure to exercise any right with respect to these Terms of Use shall be construed as a waiver by My Tribute Gift Foundation.
As a donor, you are responsible for researching the charity, understanding how your money will be used, and understanding the tax implications of your donation. We will email you a receipt, but the deductibility of your donation depends on your individual tax situation. We are not financial, tax, or legal experts. Please do not rely on our information to make decisions about your finances, taxes or legal matters. Consult a qualified professional in your jurisdiction.
Participating organizations have chosen to actively participate in the “Sponsor Program” to be represented in the Sponsor Charity array in an effort to increase their exposure and the opportunity for more memorial donations to be made to their organization. A rate structure is agreed upon and assessed on each donation processed through that program according to each organization’s individual agreement. Giving through that program has helped many organizations realize a drastic and significant increase in net donations.
If a donation form has a line item of Convenience Fee. The Convenience fee is displayed to you prior to approving the charge to your payment card. The convenience fee is in addition to the donation amount, so they do not affect the amount reaching the charitable organization. For example, a $100 donation becomes a $101.50 transaction. The convenience fee is directed immediately to a for-profit service provider. According to IRS and CRA rules the tax deductible receipt will therefore only be issued for the subtotal amount.
My Tribute Gift Foundation seeks to distribute all funds to organizations provided they have maintained active 501c3 certification in the United States or registered charity status in Canada. MTG relies on the beneficiary organizations to maintain and keep up to date contact information on file with the IRS or CRA respectively. This must be done in order for My Tribute Gift Foundation to exercise care and reasonable efforts to distribute payment promptly, without undue cost and burden to the foundation. In the event that a payment has not been deposited 12 months from issuance, the foundation reserves the right to utilize those funds in its mission to the foundation.
My Tribute Gift Foundation reserves the right to modify, change, discontinue and suspend these Terms of Use, the Website, and/or any services offered through the Website, in whole or in part, at any time, without prior notice to you. Your continued access or use of the Website after modifications to these Terms of Use have been posted on the Website shall indicate your acceptance of and agreement to be bound by these Terms of Use as modified. It is your responsibility to review these Terms of Use regularly.
