We are heartbroken to announce that our beloved father, Dino Cusinato, passed away at the age of 88, on January 3, 2024. He has now reunited in heaven with the love of his life, his wife Mary Jane, who passed away in 2007, to whom he was eternally devoted, and missed dearly. Dino was a loving husband, father, and Nonno who always put his family first and embodied the true essence of love, family, faith, and responsibility in all that he did. He lived with an unwavering commitment to his loved ones and a strong work ethic that inspired all who knew him. Dino was born in Windsor, ON, on March 8, 1935, to the late Gino and Maria Cusinato (nee Commachio), who had immigrated to Canada from Treviso, Italy, in the 1920’s. He was the cherished father of four children: Gina (John Leslie), Nicole (Steve Hrynewich), Dean (Kelly Cusinato, nee Bondy), and Laurie (Steve Brammell). He adored his seven grandchildren, Jonathan, Jared, Kaitlyn, Kirsten, Natalie, Nicholas, and Nolan. He never missed an opportunity to watch them play sports, attend field trips or help when needed. Dino was also the dear brother of Elsa Cusinato, Judge Anthony Cusinato (the late Elsie), the late Ginetta Barbaro (survived by Ron), and brother-in-law of Diane Hodut (the late Mike), the late Barbara and Bob Holmes; the late Betty and Pat Breslin; and the late Adam and his wife Rose. Dino will be lovingly and forever remembered by many nieces and nephews. Those fortunate enough to know Dino will remember him for his witty humor, vibrancy, and zest for life. Until his last day, he looked forward to his daily morning coffee at Tim’s, weekly golf games, and burgers and football talk at the Legion with his lifelong friends, as well as long dinners with his family. He could always be found manicuring his lawn and his family’s, shopping at Freed’s, walking in the mall, and attending mass at Corpus Christi Church. Many will also remember him from his lifelong career at Union Gas & Union Energy & Reliance Home Comfort. Dino’s legacy of humor, love, strength and dedication will continue to shine through his family and live on in our hearts and memories. He will forever be remembered as a loving husband, father, Nonno, and brother who enriched the lives of so many. The family would like to extend a special thank you to the Emergency Response Team, who helped us tremendously on the day of his passing, and specifically to Sergeant Thompson for his incredible compassion. If you so desire, donations to Transition to Betterness would be appreciated by the family. Visitation Monday, January 8, 2024 from 3:00 p.m.-5:00 p.m. and 7:00 p.m.-9:00 p.m. with Parish Prayers at 4:30 p.m. at Families First, South Windsor, 3260 Dougall Ave. (519-969-5841). On Tuesday, January 9, 2024, friends are invited to join the family after 8:00 a.m. until leaving in procession for a Mass of Christian Burial at 9:30 a.m. to Corpus Christi Church, 1400 Cabana Rd. My Tribute Gift Foundation makes this online charitable giving portal web page MyTributeGift.org (the “Website”) available to you subject to your acceptance of and compliance with these terms of use (the “Terms of Use”). Your donation will be processed through My Tribute Gift Foundation for distribution to your selected charity. Your credit card transaction record will appear as My Tribute Gift Foundation. An official tax receipt for your donation will be issued by My Tribute Gift Foundation. Donations are sent out to charities at the end of each month for all donations received in that previous month. So a donation made on the first day of the month would be sent at the same time as one made on last day. Payments would be sent either by paper check or direct deposit. Please note that the there is 3% + 35¢/transaction fee for credit card processing, My Tribute Gift Foundation collects a 5% administrative fee. These fees help fund the cost of processing all donations, issuing tax receipts, and sending all appropriate information to the selected charity. These fees are assumed by your selected charity, but you will receive a receipt for the full amount of your donation. There are NO additional costs the charity has to endure to participate and accept donations through MyTributeGift.org. According to the American Institute for Philanthropy (charitywatch.org) a charitable organization is considered “Good” if they are spending less than 30% on administration and fundraising costs. My Tribute Gift Foundation helps reduce those administrative costs for charities and allows donors to contribute to lasting memorials for their loved ones.
When donating online, if the donation form has an option to add a tip, there is no convenience fee. The tip is optional and can be adjusted before you submit your donation. The amount of the tip will be displayed to you before you confirm the charge to your payment card. Tips are in addition to the donation amount, so they do not affect the amount that goes to the charitable organization. Under IRS and CRA rules, tips are not tax deductible to the donor.
You warrant and represent that all information and materials you provide to My Tribute Gift Foundation, through the Website or otherwise, are accurate and truthful and do not violate third party rights. Indemnity. YOU AGREE TO INDEMNIFY AND HOLD HARMLESS THE FOUNDATION PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, ACTIONS, SUITS AND PROCEEDINGS BROUGHT BY YOU OR ANY THIRD PARTY (COLLECTIVELY "CLAIMS") ARISING OUT OF YOUR USE OF THIS SITE IN BREACH OF THESE TERMS OF USE OR THE INFRINGEMENT BY YOU OF ANY INTELLECTUAL PROPERTY RIGHTS OF ANY PERSON OR ENTITY, INCLUDING ALL LOSSES, LIABILITIES, DAMAGES, COSTS AND EXPENSES (INCLUDING ALL LEGAL FEES) ARISING FROM SUCH CLAIMS.
If in any part of these Terms of Use is held, by a court of competent jurisdiction, to be invalid or unenforceable, such part of these Terms of Use shall be eliminated to the extent permitted in that jurisdiction, for purposes of that jurisdiction, and the remaining Terms of Use shall otherwise remain in full force and effect.
These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and all federal laws of Canada applicable therein. The courts of the Province of Ontario shall have exclusive jurisdiction over any dispute arising out of these Terms of Use or your use of the Website.
These Terms of Use constitute the entire agreement between you and My Tribute Gift Foundation with respect to the Website, except as we may explicitly agree otherwise in writing.. No delay or failure to exercise any right with respect to these Terms of Use shall be construed as a waiver by My Tribute Gift Foundation.
As a donor, you are responsible for researching the charity, understanding how your money will be used, and understanding the tax implications of your donation. We will email you a receipt, but the deductibility of your donation depends on your individual tax situation. We are not financial, tax, or legal experts. Please do not rely on our information to make decisions about your finances, taxes or legal matters. Consult a qualified professional in your jurisdiction.
Participating organizations have chosen to actively participate in the “Sponsor Program” to be represented in the Sponsor Charity array in an effort to increase their exposure and the opportunity for more memorial donations to be made to their organization. A rate structure is agreed upon and assessed on each donation processed through that program according to each organization’s individual agreement. Giving through that program has helped many organizations realize a drastic and significant increase in net donations.
If a donation form has a line item of Convenience Fee. The Convenience fee is displayed to you prior to approving the charge to your payment card. The convenience fee is in addition to the donation amount, so they do not affect the amount reaching the charitable organization. For example, a $100 donation becomes a $101.50 transaction. The convenience fee is directed immediately to a for-profit service provider. According to IRS and CRA rules the tax deductible receipt will therefore only be issued for the subtotal amount.
My Tribute Gift Foundation seeks to distribute all funds to organizations provided they have maintained active 501c3 certification in the United States or registered charity status in Canada. MTG relies on the beneficiary organizations to maintain and keep up to date contact information on file with the IRS or CRA respectively. This must be done in order for My Tribute Gift Foundation to exercise care and reasonable efforts to distribute payment promptly, without undue cost and burden to the foundation. In the event that a payment has not been deposited 12 months from issuance, the foundation reserves the right to utilize those funds in its mission to the foundation.
My Tribute Gift Foundation reserves the right to modify, change, discontinue and suspend these Terms of Use, the Website, and/or any services offered through the Website, in whole or in part, at any time, without prior notice to you. Your continued access or use of the Website after modifications to these Terms of Use have been posted on the Website shall indicate your acceptance of and agreement to be bound by these Terms of Use as modified. It is your responsibility to review these Terms of Use regularly.
