Gabriel Stephen Cosentino Suddenly and tragically, Gabriel Stephen Cosentino, 37 years of age, born on May 24th 1985 in Calgary, Alberta, was taken from us and left this earth on Friday, September 9th in Whitby, Ontario. Gabriel enjoyed the outdoors and sports. Growing up he loved his camping trips with scouts and with his family all across Canada, visits to the family cottages in Ontario and Quebec, his years of hockey, baseball and swimming at Iroquois Park in Whitby, and biking and boating with his mother and stepfather. In his later years Gabriel loved to take walks, long walks. It would be nothing for him to walk ten kilometres in the morning and then decide to do it again in the afternoon. He was a consummate walker. Gabriel grew up in bilingual households and was fluent in both French and English. He attended École Secondaire Catholique Saint-Charles-Garnier in Whitby. He was a character, well-liked by his classmates, full of personality, a great sense of humour and good natured fun, he made us laugh so many times. It would be an understatement to say that we got to know his teachers, office staff and the principal very well over the years. Gabriel’s career aspirations to become a member of the RCMP dissolved, along with his brilliant sense of humour, his interests in sports and academia with the onset of schizophrenia at the age of 18. Gabriel struggled and suffered for many years, and his family with him and for him, as do one out of every one hundred Canadians and their families, with this horrible illness that robs so many of their full potential and their lives. Gabriel endured many years of hospitalization. Four months ago Gabriel was discharged to the care of his family and was provided with his own living space and supports at home and all of the freedom that he had yearned for so much, much of which had been compromised by the pandemic. We were so hopeful for him. In his last months and days he was finally happy, happy to be home, happy to be free, happy to be his own man and we were so happy for him. He is survived by his father Stephen Cosentino, his mother Nicole Lachance, his stepmother Danielle Broomfield, his stepfather Stephen Tardif, his sister Julie Cosentino, stepbrothers Danny and Sebastien Breton, his grandparents Paul and Martha Cosentino, and his numerous aunts, uncles and cousins. He is predeceased by his grandparents Jean-Baptiste and Ninette Lachance. The family would like to thank the staff who supported Gabriel and our family at Ontario Shores Centre for Mental Health Sciences, the Durham Mental Health Act team who supported and visited Gabriel daily at our home since his discharge, Durham Regional Police Services for their empathy, attention and for the resources that have been dedicated to helping us to understand the circumstances of his accidental and most tragic passing. Gabriel may be gone from this earth, but he will never be forgotten by us and he will live forever in our hearts. We loved him so very very much but now he is free at last and is walking with his namesake. A bilingual funeral Service to be held Saturday, October 1st at 1 p.m. at Assomption de Notre Dame, 384 Hillside Avenue, Oshawa. The family welcomes you to join them in a reception to be held in the hall adjacent to the church immediately following the service. Your donation will be processed through My Tribute Gift Foundation for distribution to your selected charity. Your credit card transaction record will appear as My Tribute Gift Foundation. An official tax receipt for your donation will be issued by My Tribute Gift Foundation. Donations are sent out to charities at the end of each month for all donations received in that previous month. So a donation made on the first day of the month would be sent at the same time as one made on last day. Payments would be sent either by paper check or direct deposit. Please note that the there is 3% + 35¢/transaction fee for credit card processing, My Tribute Gift Foundation collects a 5% administrative fee. These fees help fund the cost of processing all donations, issuing tax receipts, and sending all appropriate information to the selected charity. These fees are assumed by your selected charity, but you will receive a receipt for the full amount of your donation. There are NO additional costs the charity has to endure to participate and accept donations through MyTributeGift.org. According to the American Institute for Philanthropy (charitywatch.org) a charitable organization is considered “Good” if they are spending less than 30% on administration and fundraising costs. My Tribute Gift Foundation helps reduce those administrative costs for charities and allows donors to contribute to lasting memorials for their loved ones.
When donating online, if the donation form has an option to add a tip, there is no convenience fee. The tip is optional and can be adjusted before you submit your donation. The amount of the tip will be displayed to you before you confirm the charge to your payment card. Tips are in addition to the donation amount, so they do not affect the amount that goes to the charitable organization. Under IRS and CRA rules, tips are not tax deductible to the donor.
You warrant and represent that all information and materials you provide to My Tribute Gift Foundation, through the Website or otherwise, are accurate and truthful and do not violate third party rights. Indemnity. YOU AGREE TO INDEMNIFY AND HOLD HARMLESS THE FOUNDATION PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, ACTIONS, SUITS AND PROCEEDINGS BROUGHT BY YOU OR ANY THIRD PARTY (COLLECTIVELY "CLAIMS") ARISING OUT OF YOUR USE OF THIS SITE IN BREACH OF THESE TERMS OF USE OR THE INFRINGEMENT BY YOU OF ANY INTELLECTUAL PROPERTY RIGHTS OF ANY PERSON OR ENTITY, INCLUDING ALL LOSSES, LIABILITIES, DAMAGES, COSTS AND EXPENSES (INCLUDING ALL LEGAL FEES) ARISING FROM SUCH CLAIMS.
If in any part of these Terms of Use is held, by a court of competent jurisdiction, to be invalid or unenforceable, such part of these Terms of Use shall be eliminated to the extent permitted in that jurisdiction, for purposes of that jurisdiction, and the remaining Terms of Use shall otherwise remain in full force and effect.
These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and all federal laws of Canada applicable therein. The courts of the Province of Ontario shall have exclusive jurisdiction over any dispute arising out of these Terms of Use or your use of the Website.
These Terms of Use constitute the entire agreement between you and My Tribute Gift Foundation with respect to the Website, except as we may explicitly agree otherwise in writing.. No delay or failure to exercise any right with respect to these Terms of Use shall be construed as a waiver by My Tribute Gift Foundation.
As a donor, you are responsible for researching the charity, understanding how your money will be used, and understanding the tax implications of your donation. We will email you a receipt, but the deductibility of your donation depends on your individual tax situation. We are not financial, tax, or legal experts. Please do not rely on our information to make decisions about your finances, taxes or legal matters. Consult a qualified professional in your jurisdiction.
Participating organizations have chosen to actively participate in the “Sponsor Program” to be represented in the Sponsor Charity array in an effort to increase their exposure and the opportunity for more memorial donations to be made to their organization. A rate structure is agreed upon and assessed on each donation processed through that program according to each organization’s individual agreement. Giving through that program has helped many organizations realize a drastic and significant increase in net donations.
If a donation form has a line item of Convenience Fee. The Convenience fee is displayed to you prior to approving the charge to your payment card. The convenience fee is in addition to the donation amount, so they do not affect the amount reaching the charitable organization. For example, a $100 donation becomes a $101.50 transaction. The convenience fee is directed immediately to a for-profit service provider. According to IRS and CRA rules the tax deductible receipt will therefore only be issued for the subtotal amount.
My Tribute Gift Foundation seeks to distribute all funds to organizations provided they have maintained active 501c3 certification in the United States or registered charity status in Canada. MTG relies on the beneficiary organizations to maintain and keep up to date contact information on file with the IRS or CRA respectively. This must be done in order for My Tribute Gift Foundation to exercise care and reasonable efforts to distribute payment promptly, without undue cost and burden to the foundation. In the event that a payment has not been deposited 12 months from issuance, the foundation reserves the right to utilize those funds in its mission to the foundation.
My Tribute Gift Foundation reserves the right to modify, change, discontinue and suspend these Terms of Use, the Website, and/or any services offered through the Website, in whole or in part, at any time, without prior notice to you. Your continued access or use of the Website after modifications to these Terms of Use have been posted on the Website shall indicate your acceptance of and agreement to be bound by these Terms of Use as modified. It is your responsibility to review these Terms of Use regularly.
