June Hazel Warren (nee Stewardson) was born June 27, 1929 in Fort William, Ontario (now part of Thunder Bay) to William (Bill) and Irene Stewardson. June was the eldest of 2 girls. Her younger sister, Alison, predeceased her. Her father was an engineer at the Hawker Siddley Canadian Car Division in Fort William. When she was 11, her family moved to Toronto where her father took on the position of production manager on DeHaviland’s Mosquito project. June attended Victoria College, University of Toronto where she graduated with a Bachelor of Arts degree. She also met her future husband, Dr. MacDonald (Don) Robert Warren, while at university. During her university years, June worked at a summer camp near Huntsville where she had way too much fun. June taught public school in Toronto after graduation. On her marriage to Don, they moved to Ottawa where Don did his residency and June taught school. They then moved to Sunderland, Ontario, where Don went into general practice. Both of their children (David and Katherine) were born during this time. In 1957, the family moved to Fort William. June continued teaching part time and got involved with Wesley United Church and with coordinating Meals on Wheels. In 1969 the family moved to Etobicoke, Ontario where they resided until 1980. During this time, June held the position of Chair, Red Cross Homemakers Toronto. In 1980 June and Don moved to St. Thomas where June got involved in numerous initiatives. A few of these: • Sat on the board of Alma College – St. Thomas • Sat on the board of the YWCA, facilitated Sense of Self program – St. Thomas • Active in the United Church, organized conferences After Don retired, both June and Don enrolled in the Masters program at University of Western Ontario. Unfortunately, Don passed away in 1990, before he was able to complete his Masters, but June continued on and received her Masters of Theological Studies in 1994. June was a member of Central United Church in St. Thomas. June was involved in committee work and in leadership roles at every level of the Church – her own church and the United Church of Canada. United Church presbytery, mentoring, Christian education to students, bible study, pastoral care, etc. Things June loved: • Loved camping and the outdoors • Avid bridge player • Avid reader – moved to audio books when her eyesight started failing • Loved classical and gospel music • Loved to cook - pies • Loved her rose garden • Loved debating. Hated to have her integrity challenged. • Loved to travel – Caribbean, Britain, Israel, Jordan, Turkey • Loved the theatre – attended plays at Stratford from the earliest days of the Stratford Theatre. For a number of years, Don and June had season’s tickets to the Alexandra Theatre in Toronto. Also was a strong patron of the Shaw Festival. • Loved to entertain and was well known as a hostess extraordinaire • Loved to laugh June passed away peacefully at Extendicare Port Stanley on Wednesday, February 17, 2021 in her 92nd year. June is survived by her son David Warren (Lois Miles), and her daughter Katherine Warren (Judson Cox). June supported many charitable causes over the years. Arrangements have been entrusted to the Shawn Jackson Funeral Home 31 Elgin Street, St. Thomas (519-631-0570). My Tribute Gift Foundation makes this online charitable giving portal web page MyTributeGift.org (the “Website”) available to you subject to your acceptance of and compliance with these terms of use (the “Terms of Use”). Your donation will be processed through My Tribute Gift Foundation for distribution to your selected charity. Your credit card transaction record will appear as My Tribute Gift Foundation. An official tax receipt for your donation will be issued by My Tribute Gift Foundation. Donations are sent out to charities at the end of each month for all donations received in that previous month. So a donation made on the first day of the month would be sent at the same time as one made on last day. Payments would be sent either by paper check or direct deposit. Please note that the there is 3% + 35¢/transaction fee for credit card processing, My Tribute Gift Foundation collects a 5% administrative fee. These fees help fund the cost of processing all donations, issuing tax receipts, and sending all appropriate information to the selected charity. These fees are assumed by your selected charity, but you will receive a receipt for the full amount of your donation. There are NO additional costs the charity has to endure to participate and accept donations through MyTributeGift.org. According to the American Institute for Philanthropy (charitywatch.org) a charitable organization is considered “Good” if they are spending less than 30% on administration and fundraising costs. My Tribute Gift Foundation helps reduce those administrative costs for charities and allows donors to contribute to lasting memorials for their loved ones.
When donating online, if the donation form has an option to add a tip, there is no convenience fee. The tip is optional and can be adjusted before you submit your donation. The amount of the tip will be displayed to you before you confirm the charge to your payment card. Tips are in addition to the donation amount, so they do not affect the amount that goes to the charitable organization. Under IRS and CRA rules, tips are not tax deductible to the donor.
You warrant and represent that all information and materials you provide to My Tribute Gift Foundation, through the Website or otherwise, are accurate and truthful and do not violate third party rights. Indemnity. YOU AGREE TO INDEMNIFY AND HOLD HARMLESS THE FOUNDATION PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, ACTIONS, SUITS AND PROCEEDINGS BROUGHT BY YOU OR ANY THIRD PARTY (COLLECTIVELY "CLAIMS") ARISING OUT OF YOUR USE OF THIS SITE IN BREACH OF THESE TERMS OF USE OR THE INFRINGEMENT BY YOU OF ANY INTELLECTUAL PROPERTY RIGHTS OF ANY PERSON OR ENTITY, INCLUDING ALL LOSSES, LIABILITIES, DAMAGES, COSTS AND EXPENSES (INCLUDING ALL LEGAL FEES) ARISING FROM SUCH CLAIMS.
If in any part of these Terms of Use is held, by a court of competent jurisdiction, to be invalid or unenforceable, such part of these Terms of Use shall be eliminated to the extent permitted in that jurisdiction, for purposes of that jurisdiction, and the remaining Terms of Use shall otherwise remain in full force and effect.
These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and all federal laws of Canada applicable therein. The courts of the Province of Ontario shall have exclusive jurisdiction over any dispute arising out of these Terms of Use or your use of the Website.
These Terms of Use constitute the entire agreement between you and My Tribute Gift Foundation with respect to the Website, except as we may explicitly agree otherwise in writing.. No delay or failure to exercise any right with respect to these Terms of Use shall be construed as a waiver by My Tribute Gift Foundation.
As a donor, you are responsible for researching the charity, understanding how your money will be used, and understanding the tax implications of your donation. We will email you a receipt, but the deductibility of your donation depends on your individual tax situation. We are not financial, tax, or legal experts. Please do not rely on our information to make decisions about your finances, taxes or legal matters. Consult a qualified professional in your jurisdiction.
Participating organizations have chosen to actively participate in the “Sponsor Program” to be represented in the Sponsor Charity array in an effort to increase their exposure and the opportunity for more memorial donations to be made to their organization. A rate structure is agreed upon and assessed on each donation processed through that program according to each organization’s individual agreement. Giving through that program has helped many organizations realize a drastic and significant increase in net donations.
If a donation form has a line item of Convenience Fee. The Convenience fee is displayed to you prior to approving the charge to your payment card. The convenience fee is in addition to the donation amount, so they do not affect the amount reaching the charitable organization. For example, a $100 donation becomes a $101.50 transaction. The convenience fee is directed immediately to a for-profit service provider. According to IRS and CRA rules the tax deductible receipt will therefore only be issued for the subtotal amount.
My Tribute Gift Foundation seeks to distribute all funds to organizations provided they have maintained active 501c3 certification in the United States or registered charity status in Canada. MTG relies on the beneficiary organizations to maintain and keep up to date contact information on file with the IRS or CRA respectively. This must be done in order for My Tribute Gift Foundation to exercise care and reasonable efforts to distribute payment promptly, without undue cost and burden to the foundation. In the event that a payment has not been deposited 12 months from issuance, the foundation reserves the right to utilize those funds in its mission to the foundation.
My Tribute Gift Foundation reserves the right to modify, change, discontinue and suspend these Terms of Use, the Website, and/or any services offered through the Website, in whole or in part, at any time, without prior notice to you. Your continued access or use of the Website after modifications to these Terms of Use have been posted on the Website shall indicate your acceptance of and agreement to be bound by these Terms of Use as modified. It is your responsibility to review these Terms of Use regularly.
