Dr. Alan I. Lebowitz, 81, of Greenwich, CT, passed away on February 9, 2022 at home surrounded by family. Alan was truly a modern-day renaissance man who treasured the world around him and his place in it. He embraced science, technology, music, art, and literature. Alan also loved to make people laugh and he always tried to share his unique appreciation of the world. His curiosity was insatiable and his humor was a driving force. Alan enjoyed life to its fullest, maintaining his curiosity and wonder of the world throughout his life. Whether it was mastering a new song on the piano or guitar, enjoying a NY Philharmonic performance, sailing or kayaking on Long Island Sound, practicing medicine, or even taking in the view from an airplane window, Alan was always investigating, always making new discoveries, and always finding joy and fascination in everything around him. Alan was born on December 3, 1940 to Herman and Bertha Lebowitz in Philadelphia. He grew up in northeast Philadelphia, attended Northeast High School, and followed his personal dream of becoming the first physician in his family. Alan married Harriet Fisher on August 19, 1964 and cherished her and their two children, Cathy and Peter, for the rest of his life. He was also a loving and dedicated grandfather to Zoe and Eli who greatly admired and benefitted from their grandfather’s zest for life. Alan proudly served as a major in the US Army from 1969-1971 as a physician and hematologist, taking care of service members in Fort Bragg, North Carolina. After North Carolina, where Alan and Harriet made lifelong friends, the family headed to Connecticut, first in Stamford and then Greenwich. Alan practiced internal medicine and hematology in private practice in Stamford, Darien, and Greenwich for nearly 50 years. He was also an Associate Clinical Professor of Medicine at Yale University School of Medicine where he took care of hematology patients and taught students, residents, and fellows. He was loved by his patients and all the people who worked with him. He was a remarkable physician who not only cared deeply about his patients but always used the full breadth of science and medicine to seek solutions to difficult medical problems in thoughtful ways. His focus throughout his entire career was always on expanding his medical and scientific expertise to achieve excellence in providing medical care to patients with empathy and humor. After retiring, Alan helped establish a foundation in honor of his friend and colleague at Yale. The Bernard G. Forget Hematology Scholars Program was established to support research efforts of hematology residents at Yale. Alan will be missed by many but most intensely by his wife, Harriet, his two children, Cathy and Peter, his daughter-in-law, Elizabeth, and his grandchildren, Zoe and Eli. My Tribute Gift Foundation makes this online charitable giving portal web page MyTributeGift.org (the “Website”) available to you subject to your acceptance of and compliance with these terms of use (the “Terms of Use”). Your donation will be processed through My Tribute Gift Foundation for distribution to your selected charity. Your credit card transaction record will appear as My Tribute Gift Foundation. An official tax receipt for your donation will be issued by My Tribute Gift Foundation. Donations are sent out to charities at the end of each month for all donations received in that previous month. So a donation made on the first day of the month would be sent at the same time as one made on last day. Payments would be sent either by paper check or direct deposit. Please note that the there is 3% + 35¢/transaction fee for credit card processing, My Tribute Gift Foundation collects a 5% administrative fee. These fees help fund the cost of processing all donations, issuing tax receipts, and sending all appropriate information to the selected charity. These fees are assumed by your selected charity, but you will receive a receipt for the full amount of your donation. There are NO additional costs the charity has to endure to participate and accept donations through MyTributeGift.org. According to the American Institute for Philanthropy (charitywatch.org) a charitable organization is considered “Good” if they are spending less than 30% on administration and fundraising costs. My Tribute Gift Foundation helps reduce those administrative costs for charities and allows donors to contribute to lasting memorials for their loved ones.
When donating online, if the donation form has an option to add a tip, there is no convenience fee. The tip is optional and can be adjusted before you submit your donation. The amount of the tip will be displayed to you before you confirm the charge to your payment card. Tips are in addition to the donation amount, so they do not affect the amount that goes to the charitable organization. Under IRS and CRA rules, tips are not tax deductible to the donor.
You warrant and represent that all information and materials you provide to My Tribute Gift Foundation, through the Website or otherwise, are accurate and truthful and do not violate third party rights. Indemnity. YOU AGREE TO INDEMNIFY AND HOLD HARMLESS THE FOUNDATION PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, ACTIONS, SUITS AND PROCEEDINGS BROUGHT BY YOU OR ANY THIRD PARTY (COLLECTIVELY "CLAIMS") ARISING OUT OF YOUR USE OF THIS SITE IN BREACH OF THESE TERMS OF USE OR THE INFRINGEMENT BY YOU OF ANY INTELLECTUAL PROPERTY RIGHTS OF ANY PERSON OR ENTITY, INCLUDING ALL LOSSES, LIABILITIES, DAMAGES, COSTS AND EXPENSES (INCLUDING ALL LEGAL FEES) ARISING FROM SUCH CLAIMS.
If in any part of these Terms of Use is held, by a court of competent jurisdiction, to be invalid or unenforceable, such part of these Terms of Use shall be eliminated to the extent permitted in that jurisdiction, for purposes of that jurisdiction, and the remaining Terms of Use shall otherwise remain in full force and effect.
These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and all federal laws of Canada applicable therein. The courts of the Province of Ontario shall have exclusive jurisdiction over any dispute arising out of these Terms of Use or your use of the Website.
These Terms of Use constitute the entire agreement between you and My Tribute Gift Foundation with respect to the Website, except as we may explicitly agree otherwise in writing.. No delay or failure to exercise any right with respect to these Terms of Use shall be construed as a waiver by My Tribute Gift Foundation.
As a donor, you are responsible for researching the charity, understanding how your money will be used, and understanding the tax implications of your donation. We will email you a receipt, but the deductibility of your donation depends on your individual tax situation. We are not financial, tax, or legal experts. Please do not rely on our information to make decisions about your finances, taxes or legal matters. Consult a qualified professional in your jurisdiction.
Participating organizations have chosen to actively participate in the “Sponsor Program” to be represented in the Sponsor Charity array in an effort to increase their exposure and the opportunity for more memorial donations to be made to their organization. A rate structure is agreed upon and assessed on each donation processed through that program according to each organization’s individual agreement. Giving through that program has helped many organizations realize a drastic and significant increase in net donations.
If a donation form has a line item of Convenience Fee. The Convenience fee is displayed to you prior to approving the charge to your payment card. The convenience fee is in addition to the donation amount, so they do not affect the amount reaching the charitable organization. For example, a $100 donation becomes a $101.50 transaction. The convenience fee is directed immediately to a for-profit service provider. According to IRS and CRA rules the tax deductible receipt will therefore only be issued for the subtotal amount.
My Tribute Gift Foundation seeks to distribute all funds to organizations provided they have maintained active 501c3 certification in the United States or registered charity status in Canada. MTG relies on the beneficiary organizations to maintain and keep up to date contact information on file with the IRS or CRA respectively. This must be done in order for My Tribute Gift Foundation to exercise care and reasonable efforts to distribute payment promptly, without undue cost and burden to the foundation. In the event that a payment has not been deposited 12 months from issuance, the foundation reserves the right to utilize those funds in its mission to the foundation.
My Tribute Gift Foundation reserves the right to modify, change, discontinue and suspend these Terms of Use, the Website, and/or any services offered through the Website, in whole or in part, at any time, without prior notice to you. Your continued access or use of the Website after modifications to these Terms of Use have been posted on the Website shall indicate your acceptance of and agreement to be bound by these Terms of Use as modified. It is your responsibility to review these Terms of Use regularly.
